Position Overview

This full-time role focuses on enhancing community well-being through innovative support strategies aimed at individuals with disabilities and their families.

Key Responsibilities

  • Facilitate community initiatives to tackle challenges related to health, housing, employment, and social welfare.
  • Maintain confidentiality while managing sensitive information.
  • Foster a positive and supportive environment for participants and their families.
  • Monitor and document client progress effectively.
  • Evaluate community needs and available resources to optimize service delivery.
  • Assist participants in achieving their personal goals through tailored support programs.
  • Coordinate with various agencies to provide comprehensive assistance to clients.
  • Support families and educate caregivers in various settings.
  • Help young individuals navigate social, emotional, and financial challenges.
  • Engage with community groups and organizations to promote available resources.
  • Prepare necessary documentation for funding and resource allocation.

  • Patience and sensitivity to the varying needs of individuals.

Essential Qualifications

  • First Aid and CPR certification or willingness to obtain.
  • Valid driver’s license and access to a reliable vehicle.
  • National Police Check and Working with Children Check or willingness to obtain.
  • Full working rights in Australia or an appropriate visa.
  • Manual Handling Certificate or willingness to obtain.

Desirable Qualifications

  • Certificate III/IV in Disability Services, Aged Care, or related fields.
  • Other relevant qualifications that enhance service delivery.
