Key Responsibilities:
- Assist participants to achieve their identified objectives through personalized strategies.
- Develop and implement tailored programs for each individual client to ensure effective outcomes.
- Maintain accurate records of client progress and adapt interventions as necessary.
- Facilitate client engagement and empowerment through supportive relationships.
- Identify potential risks and mitigate them in accordance with established policies and procedures.
- Cultivate effective communication with clients, their families, and management team members.
- Ensure a safe and clean environment by adhering to daily task lists.
- Notify management promptly of any incidents or issues that may arise.
- Participate in scheduled training and meetings to enhance professional development.
- Support clients in managing their medications according to organizational guidelines.
- Pursue routine administrative duties, including filing, recording, checking, and balancing accounts, invoices, orders, and store requisitions.
- Provide personal care assistance to clients, including but not limited to assistance with continence aids, showering, toileting, and grooming.
- Aid clients in maintaining their homes, encompassing all aspects of domestic duties.
Requirements:
- Hold a Working with Vulnerable People Certificate.
- Possess a current National Police Certificate.
- Hold a valid Drivers' Licence.
- Have a current First Aid certificate.
- Demonstrate knowledge of the Tasmanian Disability Services Act 2012 and the National Disability Insurance Scheme Act 2013.
- Complete the NDIS Core Module.
Work Environment:
The role will involve working within Supported Independent Living settings, at client residences, and in community contexts as required.
