Senior Care Professional

Role Overview

We are seeking a highly skilled and compassionate Senior Care Professional to join our team. As a key member of our care delivery unit, you will play a critical role in providing exceptional support to elderly Australians, enabling them to maintain their independence and live life to the fullest.

Key Responsibilities
  • Deliver high-quality care planning, assessment, and coordination services to clients
  • Manage service delivery across all levels, ensuring seamless integration with government-funded programs
  • Develop, monitor, and reassess care plans in collaboration with clients, families, and healthcare professionals
  • Conduct telehealth or onsite assessments, including OH&S checks
  • Collaborate with nurses, regional managers, and other teams to enhance service offerings
  • Monitor client budgets and provide transparent fee information
  • Foster strong relationships with internal teams to facilitate client onboarding
  • Leverage influencing and partnering skills to lead and collaborate with schedulers, clinical teams, and support staff
  • Maintain compliance with relevant legislative and industry standards
  • Pursue continuous improvement initiatives across practice and service delivery
Requirements
  • Proven experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards
  • Fluency in Mandarin (speaking and writing), with Cantonese also highly desirable
  • Strong computer skills in CRM systems, database management, and Microsoft Office
  • Eligibility to work in Australia
  • National Police Clearance or willingness to obtain
