The successful applicant will also be required to undergo a current Police Check.

About the role

We are seeking an experienced Senior Care Partner to lead and support our Community Services team in delivering the Support at Home (SAH) program. This role provides leadership to Care Partners and ensures clients receive high-quality, person-centred care that supports independence, dignity, and connection to their community.

Key responsibilities
  • Lead and support Care Partners to deliver person-centred, trauma-informed, reablement-focused care.
  • Monitor care plans, client budgets, and reviews to ensure compliance with ACQSC Standards and the Aged Care Act.
  • Act as escalation point for complaints, incidents, and risks, ensuring timely resolution.
  • Mentor, coach and develop Care Partners through supervision and team meetings.
  • Drive service growth, client satisfaction and continuous improvement.
  • Ensure financial accountability and compliance with regulatory requirements.
About you
  • Diploma in Community Services or tertiary qualification in social sciences (preferred).
  • Leadership experience in aged care, community services, or a related sector.
  • Strong knowledge of aged care policy, compliance, and funding frameworks.
  • Excellent communication and relationship-building skills.
  • High financial literacy and case management experience.
  • Current Police Check, Driver's Licence, and access to a vehicle.
  • Greek language skills an advantage.
