Senior Paediatric Rehabilitation Case Manager

Our client is a leading provider of rehabilitation services, dedicated to delivering high-quality care to children and families affected by trauma. Their team of experienced case managers work closely with allied health professionals to ensure that every child or youth in their care makes progress towards their optimal recovery.
About the Role:We are seeking a highly skilled Occupational Therapist to take on a Senior Paediatric Rehab Case Manager role in Sydney. As a key member of our team, you will work collaboratively with families, children, and adolescents to assess and establish rehabilitation needs, develop personalized plans, and facilitate progress towards optimal recovery.
Key Responsibilities:- Collaborate with families, children, and adolescents to assess and establish rehabilitation needs;
- Develop and implement personalized rehabilitation plans, working closely with treating medical teams and allied health professionals;
- Support and guide families in establishing their child's rehabilitation needs and timing for different services throughout the rehabilitation continuum;
- Facilitate the development of clear, person-centred rehabilitation and life goals, and formulate rehabilitation plans for approval by the funding source;
- Develop high-quality, formalized report writing, service requests, and service costing throughout the rehabilitation program;
- Act as the key contact for all rehabilitation-related matters, including treatment, rehabilitation, care, and return to school/study for the client and their family;
- Ongoing monitoring of progress, reporting, and identification of barriers to recovery throughout the rehabilitation continuum, with problem-solving in collaboration with all involved to overcome obstacles and manage issues to enable the child/young person to move forward.
- $105 - $115K + super;

We are seeking an experienced Occupational Therapist with a strong background in paediatric rehabilitation and case management. The ideal candidate will have:
Degree in Occupational Therapy with AHPRA registration;Minimum 5 years clinical experience;Demonstrated ability to work effectively with children, adolescents, and families;Understanding of traumatic brain injury and its effects on a child/adolescent;Experience working with children/adolescents who have suffered traumatic injuries or have complex disabilities;Knowledge of the Compulsory Third Party (CTP) and Lifetime Care and Support scheme and a willingness to be an Approved Case Manager (Lifetime Care and Support);Demonstrated ability to work within an interdisciplinary team;Excellent verbal and written communication skills, including complex negotiation skills;Current unrestricted driver's license;Eligible to work permanently in Australia.-
