About the Role:

We are seeking a talented Physiotherapist to take on a Senior Paediatric Rehab Case Manager role. As a key member of our team, you will work collaboratively with families, children, and adolescents to assess and establish their rehabilitation needs. Your expertise will be instrumental in developing personalized rehabilitation plans, facilitating communication between healthcare professionals, and ensuring seamless care coordination.

Key Responsibilities:
  • Collaborate with families, children, and adolescents to assess and establish rehabilitation needs.
  • Develop and implement personalized rehabilitation plans, taking into account the unique needs and goals of each individual.
  • Facilitate communication between healthcare professionals, ensuring seamless care coordination and minimizing barriers to recovery.
  • Provide expert guidance and support to families, empowering them to take an active role in their child's rehabilitation.
  • Monitor progress, identify areas for improvement, and adjust rehabilitation plans as needed to ensure optimal outcomes.
Requirements:
  • Degree in Physiotherapy with AHPRA registration.
  • Minimum 5 years of clinical experience, with a strong background in paediatric rehabilitation.
  • Excellent communication and interpersonal skills, with the ability to work effectively with children, adolescents, and families.
  • Knowledge of traumatic brain injury and its effects on children and adolescents.
  • Experience working with children and adolescents who have suffered traumatic injuries or have complex disabilities.
  • Understanding of the Compulsory Third Party (CTP) and Lifetime Care and Support scheme, with a willingness to be an Approved Case Manager (Lifetime Care and Support).
  • Ability to work within an interdisciplinary team, with excellent negotiation and problem-solving skills.
