Key Responsibilities:

  • Support families, caregivers, and clinical staff to understand how dementia impacts each individual.
  • Equip carers and clinicians to prevent and respond to behaviour changes through modelling, coaching, and development of resources including behaviour support planning.
  • Develop reports that are contextualised to individual situations and people, effecting change on the ground and improving quality of life for people living with dementia.
  • Undertake a range of assessments to understand the holistic experience of the person and their care network.
  • Work in partnership with the broader team to build capability across the home and aged care sectors.
  • Contribute to research, projects, and programs, working innovatively to meet the changing needs of the sector.

Requirements:

  • Experience supporting people living with dementia – an understanding of dementia and a person-centred approach to care, with a genuine commitment to supporting individuals and their families.
  • Relevant qualification in Allied Health, Nursing, Aged Care or Mental Health is essential.
  • Excellent problem-solving and decision-making skills, high level of negotiation, communication, and relationship building skills.
  • Ability to prioritise multiple tasks and meet deadlines, excellent written and verbal communication skills.
