Job Description

Key Responsibilities:
  • Deliver a wide range of supports assisting people with disability and mental health needs as they transition from hospital to home.
  • Support customers to engage in community-based activities.
  • Support person-centred plans that identify a customer's life goals.
  • Ensure positive behaviour support plans are followed.
  • Administration duties, expense and mandatory reports.
Requirements:
  • Extensive experience in supporting customers with behavioural support plans.
  • Extensive experience working with challenging behaviours.
  • Previous experience with manual handling and medication administration.
  • Significant personal care and domestic duties experience.
  • Ability to manage situations as they arise with patience and maturity.
  • Certificate in disability or mental health or higher.
  • A valid driver's license and a willingness to use your private vehicle.

This role requires you to have strong communication skills and empathy for individuals with disabilities.
