If you're ready to create positive change every day, this could be the perfect role for you A Day in the Life: Coordinate tailored Home Care Package (HCP) plans and manage care delivery, budgets, and clinical support to ensure holistic, person-centred care at home.
Provide nursing care one day per week, including assessments, treatments, and care planning as part of the Wellbeing Manager role.
What We Offer: Salary: $92,861 - $101,835 + 12% super (based on experience)Perks: Salary packaging ($15,899) + meal/entertainment card, travel allowance or vehicle, phone/tools provided Support & Growth: Admin support, paid parental leave (9 weeks), and career development opportunities About You: MUST HAVE Current Registered Nurse (AHPRA) with hands-on experience and a passion for supporting older adults to live independently Strong case management skills (ideally in community/aged care), HCP assessment experience, and a goal-oriented, holistic approach Excellent communication and stakeholder engagement, valid driver's licence, and willingness to travel locally Successful applicants must complete a criminal history check and show proof of work rights in Australia.
