A Day in the Life

• Coordinate tailored Home Care Package (HCP) plans and manage care delivery, budgets, and clinical support to ensure holistic, person-centred care at home.
• Provide nursing care one day per week, including assessments, treatments, and care planning as part of the Wellbeing Manager role.

What you bring

• Strong case management skills (ideally in community/aged care), HCP assessment experience, and a goal-oriented, holistic approach
• Excellent communication and stakeholder engagement, valid driver's licence, and willingness to travel locally
• Current driver's licence and access to a registered, insured vehicle
• Willingness to travel up to 45 - 60 minutes
• MUST HAVE Current Registered Nurse (AHPRA) with hands-on experience and a passion for supporting older adults to live independently
• Successful applicants must complete a National Police Check and provide proof of work rights in Australia
• Full COVID-19 vaccination highly desirable
