Key Responsibilities:- Support clients to participate in age-appropriate activities and tasks, including employment, in-home support, and community access.
- Provide direct assistance to each client in accordance with their service plan and as directed.
- Work in accordance with medication administration policies, work health and safety procedures, and client rights policies, including privacy and confidentiality.
- Monitor the safety and well-being of clients and report any concerns to the Manager.
- Ensure completion of appropriate documentation, including recording relevant information on support provided in client notes and communication books.
- Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
- Work with clients, family/carers, relevant case managers, and other professionals to support clients' individual goals.
- Assist in developing, implementing, and evaluating training and behavior programs to attain identified individual goals.
- Ensure all programming undertaken is appropriately maintained, documented, and reviewed.
- Assume the role of key worker for designated clients, including preparing a monthly report on client progress.
- Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.
- Report regularly to line managers or supervisors.
- Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes, and policy review.
- Provide backup and support to other staff, as necessary.
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Notify the service if unable to work rostered shifts, with as much notice as possible.
- Contribute to Case reviews with Employment Consultants and Facilitators.
- Take reasonable care for their own health and safety.
- Take reasonable care that their acts or omissions do not adversely affect the health and safety of other persons.
- Report any quality and safety incidents or hazards at work to their manager or supervisor.
- Passport with valid visa proof.
- Police Check (less than 6 months old).
- Working with Children's Check (for paid employment).
- Current NDIS Check.
- Current First Aid Certificate & CPR.
- Certificate III in Individual Support or equivalent.
- NSW Driver's License (only required for Disability Support Workers).
- Full working rights within Australia.
- Effective oral and written communication in English.
- Experience preferred in the disability field and/or aged care.
- Ability to write comprehensive reports and other administrative tasks.
- Mandatory Training:
- Medication Management.
- Infection Control.
- NDIS Worker Orientation Module (NDIS Code of Conduct).
- Manual Handling for Healthcare Workers.
- Flexible hours available.
- No experience is required for this role.
- A work visa can be provided for this role.
- Expected salary: $65,000 - $75,000 per year.
