Job Summary

We are seeking a highly skilled and compassionate Support Worker to join our team at Lifestyle Solutions. As a Support Worker, you will play a vital role in providing person-centered support to individuals with complex needs, enabling them to live independently and participate fully in their communities.

Key Responsibilities
  • Mentor and support individuals to develop their skills and interests
  • Assist with daily living tasks, such as cooking, domestic duties, and personal care
  • Administer medication and adhere to individual support plans
  • Complete accurate and timely handover notes and incident reports
  • Provide transportation and support to individuals to access community programs and activities
Requirements
  • Current Certificate IV in Disability or equivalent
  • Experience working with individuals with complex needs
  • Ability to work a variety of shifts, including mornings, afternoons, evenings, and weekends
  • Current driver's license and NDIS Worker Screening Clearance
  • First Aid and CPR certificates
