Lead Weekend Patient Flow

The position is responsible for leading the weekend patient flow to ensure safe, timely discharges at a tertiary health care centre. Collaboration with an interprofessional team is key to delivering high-quality clinical pharmacy services.

Key Responsibilities:
  • Provide evidence-based clinical pharmacy services to inpatients
  • Act as a clinical reference point within the multidisciplinary team
  • Support and mentor junior pharmacists, students, and assistants
  • Contribute to service improvement initiatives and quality use of medicines
  • Participate in weekend rosters to enhance timely patient discharges
Requirements:
  • Bachelor of Pharmacy and AHPRA registration are mandatory
  • Proven ability to manage complex inpatient caseloads and prioritise discharges is essential
  • Strong communication skills and ability to work within a multidisciplinary team are required
  • Experience supervising and training junior staff and students is beneficial
