A care coordinator role is available in a home care setting. This position involves coordinating, managing, and administering the delivery of care services to clients.
The ideal candidate will have experience with managing client enquiries, client consultations, client onboarding, and a client case load for Home Care Package, NDIS, and private pay clients. The care coordinator will have experience managing the complex scheduling requirements for community care workers.
- Scheduling services for clients by rostering care workers dependent on their qualifications and availability.
- Meeting with clients and assessing their needs.
- Preparing in-home care assessments and drafting care plans to best meet client requirements.
- Recruiting, training, and managing the performance of the care team.

Requirements- Certificate III in Individual Support (Ageing, Disability) or equivalent.
- Current Australian National Police check, Working with Children Check, and Current First Aid & CPR certificate.
