Physiotherapist and Case Manager Role

This is an exciting opportunity to join a multidisciplinary team as a Physiotherapist and Case Manager, working with adults with traumatic brain injury.

The role involves delivering interventions and case management in a variety of settings, including clinics, outreach programs, community residential care, and telehealth. You will work collaboratively with other healthcare professionals to provide holistic care and support for clients to achieve their goals.

About the Role

As a Physiotherapist and Case Manager, you will be responsible for:

  • Delivering impairment-based and functional interventions to support community reintegration;
  • Working with clients to identify and address their needs and goals;
  • Collaborating with other healthcare professionals to develop and implement comprehensive treatment plans;
  • Providing education and support to clients and their families;
  • Maintaining accurate records and reporting requirements.
Requirements

To be successful in this role, you will need:

  • Tertiary qualifications in Physiotherapy and eligibility for membership of Australian Physiotherapy Association;
  • Current eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve driving long distances.
