Role Overview

We are seeking a skilled and experienced Clinical Coordinator to join our team. This role involves assessing, coordinating, and managing patient admissions in a Hospital in the Home (HITH) setting.

As a Clinical Coordinator, you will operate under a care coordination model, liaising with multidisciplinary teams, community organisations, and healthcare providers to ensure efficient and effective patient management and discharge planning.

A key responsibility of this role is promoting the HITH service within the organisation and to external stakeholders. You will also support innovation and efficiency in working practices, ensuring excellence in patient care.

Qualifications And Experience Required
  • Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
  • Desirable experience in acute and ambulatory/community settings
  • Current Victorian Driver's Licence
