We are seeking a motivated and caring individual to join our community as a Wealth Enhancer. This is a permanent, full-time position based onsite at our community.
As a Wealth Enhancer, you will play a vital role in championing the health, wellbeing, and connectedness of our residents. You'll work closely with residents and their families, as well as internal and external partners, to support individual wellness goals and facilitate access to tailored services such as Home Care, health education, social activities, and aged care planning.
- Support residents in developing and reviewing personalized Wellbeing Plans.
- Connect residents with appropriate services, including home care, aged care, and community health resources.
- Partner with the Resident Social Committee to deliver monthly activity calendars and community events.
- Assist new residents during their transition into village life, ensuring a smooth and welcoming experience.
- Maintain accurate wellbeing and emergency information using our systems and processes.
- Collaborate with internal stakeholders and external providers to improve service delivery and resident satisfaction.

Key ResponsibilitiesProviding Support ServicesYou will provide support to residents in developing and reviewing their Wellbeing Plans, connecting them with relevant services, and partnering with the Resident Social Committee to deliver community events.
