This position is based across our Fitzroy, East Melbourne, Kew, and Werribee campuses. This is a casual role designed to provide support to our existing social work team.

As an outstanding Social Worker, you will bring:

3. POSITION PURPOSE
To ensure that the complex social needs of patients at St Vincent's Private Hospital Melbourne is provided in a person centred care manner, within the vision, mission and values of SVPHM

Ensure that the confidentiality of patients, their family and staff is maintained and communication and interpersonal skills are effective and appropriate.
To work within your scope of practice to provide social work support to patients with complex social requirements.

The provision of social work support to patients at SVPHM may include generic social work, assistance to families when selecting Aged Care facilities, education and advocacy to patients and family members when required and counselling to patient and family members in regards to grief, relationship issues, stress management and mild legal issues such as participation in the organisation of enduring powers of attorney.

4. KEY ACCOUNTABILITIES
Position and Accountability

- Provide medical focused social work in a Hospital inpatient setting
- An understanding of the range of issues affecting people of Aboriginal and Torres Strait Islanders
- Working with the multidisciplinary team to assist in the discharge of patients with complex social issues which may involve the management and implementation of the following:
- Participation in the Aged Care Assessment Service (ACAS) assessment process
- Management of the administrative tasks for that process
- Assistance in the development of a discharge plan consistent with the patient's assessment by members of the patient's treatment team, the patient and the patient's carer
- Ensuring that the discharge plan is properly implemented in a timely manner
- Development of community resources to make sure suitable discharge options are available for the patient
- Assists staff to identify patients with complex social issues early during their hospital episode.
- Participate in the Multi-Disciplinary Team meetings if these occur on Campus

5. SELECTION CRITERIA

5.1 KNOWLEDGE AND EXPERIENCE REQUIRED:

- Considerable Hospital experience working as a Social Worker
- Eligible for registration with the Australian Association of Social Workers
- Strong knowledge of the Aged Care sector
- Possess a clear understanding of General Medical social work and General Medical assessments
- Provide evidence based practice in accordance with AASW guidelines and regulations
- Have minimum Bachelor of Social Work qualifications
- A valid National Police Clearance and Working with Children Clearance

5.2 PRACTICAL/SPECIALIST SKILLS:

- Excellent written and verbal communication skills
- Demonstrated leadership skills

- High level computer literacy

ROLE DESCRIPTION
The role of the Social Worker at St Vincent's Private Hospital Melbourne is to provide support to patients with complex social issues across acute and subacute settings. These settings will include General Medical, Oncology, Renal, Gastroenterology, Cardiac, Maternity wards and interaction with the Palliative Care Team.

The Social Worker is part of each ward's team, working closely with Nursing, Discharge Coordinator, Medical and Allied Health teams. Your focus will be managing patients and their care providers out of hospital, in a safe and risk free manner. You will assist patients to manage their care and to support their needs during their inpatient stay and assist in the discharge planning of the patient utilising community resources to support the management plan. You will be working autonomously and as part of a team and you will be guided by your duty of care and the vision and values of St Vincent's Private Hospital Melbourne.
Social Work involvement is dependent upon referral from medical or nursing staff. The Social Worker is a member of the Continuum of Care Team and reports to the Continuum of Care Coordinator.
