Job Description

The role of a Social Worker in the Cardiac Heart Failure and Cardiology Service at Alfred Health involves providing advanced social work services to patients and their families. This includes comprehensive assessments, support, and guidance through transitions from hospital to home.

The Social Worker will be responsible for working with patients who require complex care, including those with heart failure and transplant patients. The role requires strong collaboration and communication skills, as well as the ability to work effectively in a team environment.

Key Responsibilities:

  • Provide comprehensive assessments and develop individualized care plans for patients

  • Support patients and their families through hospitalization and discharge

Required Skills and Qualifications

To be successful in this role, you will need:

  • A Bachelor of Social Work or recognized equivalent
  • Membership of the Australian Association of Social Workers
  • Effective team collaboration and family engagement skills
  • Experience or knowledge in Family Violence, Child Protection, NDIS, or related areas
  • Knowledge of hospital social work legislation and policies
