Location: across St Vincent's Private Hospital Melbourne campuses in Fitzroy, East Melbourne, Kew and Werribee. This is a casual role designed to provide support to the existing social work team.

About the Role

Role: Social Worker in the Continuum of Care Department. The Social Worker is part of each ward's team, working with Nursing, Discharge Coordinators, Medical and Allied Health staff to support patients with complex social needs and assist in discharge planning using community resources. Work may be required across acute and subacute settings including General Medical, Oncology, Renal, Gastroenterology, Cardiac and Maternity wards, in collaboration with the Continuum of Care Team.

Responsibilities
  • Provide medical-focused social work in a hospital inpatient setting.
  • Collaborate with multidisciplinary teams to discharge patients with complex social needs.
  • Participate in the Aged Care Assessment Service (ACAS) assessment process and manage related administrative tasks.
  • Develop and implement discharge plans aligned with the patient's assessment, including engagement with carers and families.
  • Coordinate community resources to ensure suitable discharge options.
  • Identify patients with complex social issues early and participate in Multi-Disciplinary Team meetings as needed.
Qualifications & Requirements
  • Bachelor of Social Work (minimum).
  • Proven hospital experience as a Social Worker.
  • Eligible for registration with the Australian Association of Social Workers (AASW).
  • Strong knowledge of the Aged Care sector and General Medical social work.
  • Provide evidence-based practice in line with AASW guidelines.
  • A valid National Police Clearance and Working with Children Clearance.
  • Knowledge of issues affecting Aboriginal and Torres Strait Islander peoples.
Requirements & Compliance
  • Criminal record and/or child protection checks will be conducted as part of the selection process.
  • Compliance with SVHA Staff Health Screening Immunisation Policy, including mandatory immunisations/vaccinations as required by role (including flu).
