Social Services Coordinator

The Northern Adelaide Local Health Network has an exciting opportunity for a dedicated and enthusiastic Social Worker to join their Geriatrics Service. As a key member of the multidisciplinary team, you will play a vital role in supporting older adults and their families during challenging times. Your expertise in assessment, counselling, and advocacy will be invaluable in ensuring clients receive the care and support they need.
About the Role:
Key responsibilities include conducting comprehensive psychosocial assessments, developing care plans tailored to individual needs, providing counselling and support to clients and their families, collaborating with multidisciplinary teams, advocating for clients' rights, and facilitating support groups and community education programs.
