Transition Care Program Coordinator

About the Role:
The Northern Adelaide Local Health Network is seeking a skilled Transition Care Program Coordinator to support patients and their families during the transition period between hospital and home or residential aged care.
As a Transition Care Program Coordinator, you will play a vital role in coordinating and facilitating the efficient discharge of eligible clients, ensuring they receive the necessary support and services to continue their recovery journey in a comfortable and familiar environment.
Key responsibilities include collaborating with multidisciplinary teams, clients, and their families to ensure a seamless transition experience. You will also have the opportunity to contribute to the continuous improvement of the Transition Care Program, ensuring it remains responsive to the evolving needs of our community.
To excel in this role, you will possess exceptional communication and organisational skills, empathy, and the ability to navigate complex situations with sensitivity.
Requirements:
- Appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service:
- Occupational Therapist with the Occupational Therapy Board of Australia
- Physiotherapy Board of Australia
- Speech Pathology Australia
- Exercise Sports Science Australia (ESSA)
- Australian Association of Social Workers
- Pharmacy Board of Australia
