We're seeking a dedicated individual to deliver comprehensive support to clients engaged in our program. As a Health Linker, you will collaborate with clients and various stakeholders to streamline access to essential services, creating personalized care plans that cater to their unique needs.

Key Responsibilities:

  • Implement holistic, coordinated support to enhance client health and well-being.
  • Collaborate with clients to craft tailored support plans, connecting them to vital health and social services.
  • Help individuals tackle broader challenges affecting their health, such as financial difficulties, housing issues, unemployment, isolation, and caregiving responsibilities.
  • Provide guidance to clients, their families, and caregivers to boost health literacy and promote healthy lifestyle choices.

Requirements:

  • Relevant tertiary qualifications in Nursing, Aboriginal Health, Allied Health, Community/Human Services, Social Work/Social Science, or equivalent.
  • A solid understanding of the broader determinants of health—including social, economic, and environmental factors—and their impact on communities.
  • Proven experience in co-developing person-centred support plans that prioritise client well-being and aspirations.
  • Advocacy skills to enhance client access to programs and inspire community participation.
  • Exceptional communication and interpersonal abilities, particularly with culturally and linguistically diverse (CALD) backgrounds and First Nations communities.
