About the Role

We are seeking a dedicated and compassionate Care Coordinator to join our team and play a vital role in supporting elderly individuals in the community to live independently and with dignity in their own homes.

In this role, your primary responsibility will be to manage a caseload of community-based elderly individuals with Home Care Packages. You will assess their needs, develop personalized care plans, and manage budgets in accordance with program guidelines, to ensure each customer's unique requirements are met. You'll also onboard new customers and proactively manage existing ones with a person-centered approach. Collaboration with our Clinical, Scheduling, and Service Delivery teams will be crucial to delivering comprehensive care.

In this permanent full-time role, you will have the flexibility to manage your own schedule each week while working autonomously from home and within the community. You will visit customers in their homes across Wollongong and the Sutherland Shire regularly, therefore you will ideally be located in or close to this region.

About You
  • Previous experience in community-based case management is desirable
  • Experience with Home Care Packages is desirable
  • A current driver's license is required
  • A reliable vehicle with minimum Third Party insurance is required
  • Strong communication skills, both verbal and written, are essential
  • Demonstrated empathy and compassion are required
  • Excellent problem-solving abilities and critical thinking skills are necessary
  • Good organizational and time management skills are required
  • Cultural competence and the ability to work with diverse populations are essential
  • Advocacy skills to ensure clients' needs are met are required
  • Collaboration and teamwork skills to work effectively with interdisciplinary teams are necessary
