About the Role

This Transitional Housing Worker position offers an exciting opportunity to make a real difference in the lives of young people. You will work with a leading provider of youth homelessness services in Western Sydney, supporting young people to achieve independent living and overcome challenges.

Key Responsibilities
  • Build and maintain strong relationships with young people, working towards their individualised goals and objectives.
  • Provide comprehensive support in areas such as education, employment, and community engagement, tailored to each client's needs.
  • Collaborate with Personal Advisors and external agencies to ensure a cohesive and effective support network for clients.
  • Manage a caseload, ensuring accurate documentation and case plans are up-to-date and aligned with organisational policies.
  • Prioritise ongoing professional development and contribute to policy reviews, team meetings, and knowledge-sharing initiatives.
What We're Looking For
  • A minimum of two years' experience working with young people at risk of or experiencing homelessness, with a strong understanding of out-of-home care systems and transitional housing programs.
  • Demonstrated expertise in trauma-informed practice and client-centric service delivery, with excellent communication and interpersonal skills.
  • The ability to work autonomously and collaboratively as part of a team, with a flexible and adaptable approach to meeting client needs.
Requirements and Qualifications
  • A minimum Diploma in Youth Work, Community Services, or a related field (or equivalent work experience).
  • An unrestricted current 'C' Class driver's licence, valid National Police Check, and Working with Children Check.
  • A current First Aid Certificate and CPR Certification, with Australian citizenship or permanent residency.
