Social Inclusion Specialist

We are seeking a skilled professional to deliver case management services to clients experiencing chronic homelessness as part of the Journey to Social Inclusion (J2SI) program.
The J2SI program provides long-term, personalized support to help individuals access stable housing, build life skills, and foster independence using a trauma-informed approach.
Key Responsibilities:
- Deliver comprehensive case management services to vulnerable populations
- Work collaboratively with multidisciplinary teams to provide wrap-around support
- Develop and implement individualized plans to achieve client goals
- Maintain accurate records and reporting
