Disability Practitioner Role

We are seeking a skilled professional to join our interdisciplinary team as a Disability Practitioner. In this dynamic role, you will play a vital part in helping individuals enjoy everyday life by understanding their unique needs and preferences.

About the Role:

This is an exciting opportunity for a social worker to use evidence-based practice and professional reasoning to recommend the most suitable supports, therapy, and services for participants. You will work collaboratively with a dedicated team of practitioners, habit coaches, and other stakeholders to empower individuals to achieve greater outcomes.

Your Key Responsibilities:

  • Undertake initial assessments using the LEAD tool to identify participant needs and develop personalized plans
  • Support individuals in their natural environments, such as day programs or homes, to bring their hopes and dreams to life
  • Attend peer group supervision/mentoring and internal supports to enhance your expertise and skills
  • Participate in weekly team meetings to share ideas, solutions, and contribute to team growth
  • Develop effective working alliances with participants and their families to identify factors affecting a working alliance and repair breakdowns in a timely manner

Requirements:

  • Qualification in Social Work and full membership of the Australian Association of Social Workers
  • Minimum 12 months experience working as a social worker
  • Passion for empowering individuals with disabilities to achieve greater outcomes
