Key Responsibilities- Develop and implement individualized care plans that meet the unique needs and preferences of our clients.
- Provide ongoing care management for Home Care Packages, conducting regular reviews of client needs and goals, and making adjustments to care plans as necessary.
- Liaise and advocate with other service providers to ensure seamless care coordination and support.
- Collaborate with multidisciplinary teams to provide comprehensive care and support services to our clients.
- Minimum of 3 years' experience in Home Care Packages program and/or tertiary qualification in Social Work, Allied Health, Nursing, Care Management, or equivalent.
- Extensive experience working with older people and/or with Home Care Packages consumers.
- Demonstrated experience in Consumer Directed Care (CDC) choices and preferences to consumers.
- Knowledge of aged care related specialist services, such as Parkinson's and Dementia support Services, would be an advantage.
- Knowledge of budget management and fees and costs associated.
