About the Role

We are seeking a highly skilled and compassionate Care Manager to join our multidisciplinary team at Community Health Service Ltd. As a Care Manager, you will play a vital role in providing quality case management services to our clients, ensuring they receive the best possible care and support to achieve their goals and remain living at home and in their community.

Key Responsibilities
  • Develop and implement individualized care plans that meet the unique needs and preferences of our clients.
  • Provide ongoing care management for Home Care Packages, conducting regular reviews of client needs and goals, and making adjustments to care plans as necessary.
  • Liaise and advocate with other service providers to ensure seamless care coordination and support.
  • Collaborate with multidisciplinary teams to provide comprehensive care and support services to our clients.
Requirements
  • Minimum of 3 years' experience in Home Care Packages program and/or tertiary qualification in Social Work, Allied Health, Nursing, Care Management, or equivalent.
  • Extensive experience working with older people and/or with Home Care Packages consumers.
  • Demonstrated experience in Consumer Directed Care (CDC) choices and preferences to consumers.
  • Knowledge of aged care related specialist services, such as Parkinson's and Dementia support Services, would be an advantage.
  • Knowledge of budget management and fees and costs associated.
