We're seeking a dedicated individual to provide exceptional support to our clients, empowering them to live with dignity and independence.

This role involves delivering high-quality personal care, including assisting with daily living skills, managing medication, and facilitating community participation.

The ideal candidate will possess strong interpersonal skills, be able to work collaboratively with the clinical team, and demonstrate a commitment to ensuring treatment plans are followed.

Key Responsibilities:
  • Client Support: Assist participants with daily living activities, such as personal hygiene, meal preparation, and transportation.
  • Care Planning: Collaborate with the clinical team to develop and implement personalized care plans that address the unique needs of each client.
  • Behavioral Support: Implement intervention techniques and provide psychosocial supports to help clients manage challenging behaviors.
  • Data Entry: Accurately record client progress and challenges in our system, providing valuable insights for future care planning.
Requirements:
  • A certificate in Individual Support or Aged Care is required.
  • 6 months experience working in a similar role.
  • Availability during the week and on weekends.
  • National Disability Insurance Scheme (NDIS) Workers Check.
  • Valid driver's license and car with comprehensive insurance.
  • Working with Children Check.
  • Excellent communication and interpersonal skills.
  • Maintain impeccable presentation at all times.
  • Experience in managing clients with complex needs.
