Job Title

We are seeking a skilled professional to lead our Rehabilitation Access Coordinator team, ensuring seamless referrals and assessments for patients.

Main Responsibilities:

  • Collaborate with internal and external stakeholders to optimise rehabilitation referrals.
  • Prioritise and complete referrals in a timely manner.
  • Provide clear communication regarding the plan of care to key stakeholders.
  • Foster teamwork within the RAC team and develop a customer service culture.
  • Conduct comprehensive assessments on referred patients and determine their current and ongoing service needs.
  • Develop and communicate the rehabilitation plan of care to the patient, their family, and the healthcare team.

Requirements:

  • A tertiary qualification in a related clinical health profession (Allied Health or Nursing).
  • Current FIM credentialing or work towards within 3 months of commencement.
  • A current Victorian Driver's License.
  • Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Recent clinical experience within rehabilitation.
  • Leadership and/or management experience.
  • Demonstrated understanding and commitment to delivering patient-centred care.
