Your Opportunity
As a Home Care Support Professional, you will be responsible for providing high-quality care and support to our residents in their own homes. You will work collaboratively with our multidisciplinary team to deliver holistic care that meets the individual needs of each resident.
Key Responsibilities
- To provide personal care and support to residents in their own homes, including assistance with daily living activities such as bathing, dressing, and grooming.
- To assist residents with their medication, meal preparation, and other daily tasks as required.
- To engage with residents in meaningful conversations and activities, promoting social interaction and emotional well-being.
- To maintain accurate records of resident interactions, including care plans and progress notes.
- To work collaboratively with other healthcare professionals to ensure seamless care transitions and optimal outcomes.

Requirements
- A Certificate III or higher in Individual Support or a related field.

- Physical ability to perform duties in a fast-paced environment, including lifting and transferring residents as required.
- Ability to maintain confidentiality and handle sensitive information with discretion.
