About the Role
  • Undertake in-depth client assessments for intake and provide information and referral as needed.
  • Develop holistic individual case plans that encompass strength-based and solutions-focused interventions in collaboration with clients to address their needs and goals.
  • Create realistic housing plans for clients participating in the program to facilitate informed decision-making.
  • Facilitate an Accommodation Living Skills Program for individuals experiencing homelessness.
  • Prepare reports for the Team Leader or Manager as requested.
