About the Role:

This role involves providing clinical and non-clinical home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). You will work with clients and their representative/s to establish goals specific to their needs and develop appropriate strategies and actions.

Requirements:

  • Mandatory qualifications (Nursing or Allied Health) and proof of registration (if applicable).
  • This is a Vaccine Preventable Disease (VPD) risk role.
  • Possession of class C drivers' licence.
  • Travel is a requirement of this position.
  • Probation period may apply.
  • Pre-employment checks will be conducted including criminal history clearance.
