Clinical Care Coordinator Role

  • Key Responsibilities:
  • Prioritize resident needs, ensuring timely and tailored care provision by clinical staff.
  • Manage AN-ACC reappraisals, including mandatory and voluntary assessments, to ensure quality care.
  • Hold nurses accountable for daily case reporting, care planning, and addressing patient concerns promptly.
  • Conduct routine clinical assessments to address gaps in care and improve outcomes.

About the Role:

In this role, you will be responsible for leading clinical governance, managing specialized portfolios, and promoting high-quality care delivery.

Requirements:

  • Demonstrated senior management experience in aged care.
  • Registered as a Nurse with AHPRA, with no restrictions.
  • Strong knowledge of the aged care industry and best practices.
  • Proficiency in HR matters and negotiation skills.
  • Skilled in utilizing information technology tools and software.
