As a dedicated Care Services Coordinator, you will play a vital role in ensuring our clients receive high-quality home care services tailored to their specific needs. Your expertise in case management and client advocacy will contribute to improving the quality of life for those in our care.
- Develop and implement care plans tailored to each client's care needs.
- Conduct client assessments, including initial intake and ongoing reviews.
- Coordinate and monitor the delivery of home care services.
- Liaise with clients, families, caregivers, and external service providers.
- Ensure compliance with regulatory standards and organisational policies.
- Advocate for clients to access necessary resources and services.
- Maintain accurate and up-to-date case notes and documentation.
- Respond promptly to client and family concerns or changes in circumstances.
