Transition and Empowerment Role

This exciting role will see you working closely with young people aged 16-21, supporting them as they transition towards independence.

You will assist in the development of life skills, including budgeting, meal preparation, household management, and understanding housing needs.

About The Role
  • As a Transition and Empowerment Youth Worker, you will provide secondary consultation to the care team, lending expertise to the transition planning process and identifying goals for inclusion in the 15+ Care and Transition Plan.
  • You will also provide case work support while the young person is subject to a statutory order and as they move towards independence use assertive outreach in engagement.

This is an excellent opportunity for someone who is passionate about supporting vulnerable young people and working collaboratively with families, colleagues, government departments, sector partners and the community.

Key Responsibilities
  • Provide guidance and support to young people aged 16-21 as they transition towards independence.
  • Assist in the development of life skills, including budgeting, meal preparation, household management, and understanding housing needs.

Requirements
  • Qualifications in youth work, community services, child youth and family.
  • Valid Police Check.
  • Working with Children's Check for a paid employee (or willingness to obtain).
  • Victorian Driver Licence.
  • Current COVID-19 vaccination (Strongly Recommended).
