Job Opportunity

We are seeking a skilled and compassionate Social Worker to join our healthcare team. The successful candidate will have the confidence and ability to work independently and collaboratively with patients, families, and other professionals to provide high-quality care.

Main Responsibilities:
  • Conduct thorough assessments of patient needs and develop individualised care plans in collaboration with the multidisciplinary team.
  • Provide leadership and support to patients, families, and carers to enhance their recovery and well-being.
  • Identify opportunities for prevention and intervention through risk assessment and develop effective strategies to promote patient safety.
Requirements:
  • A minimum of a Bachelor's degree in Social Work or an equivalent qualification is required.
  • The ideal candidate will have proven experience working as a Social Worker in a mental health setting.
  • Excellent communication and interpersonal skills are essential, with the ability to engage with patients, families, and carers from diverse backgrounds.
  • A commitment to quality improvement and evidence-based practice is necessary.
