Clinical Nurse Specialists are crucial in delivering high-quality patient care, ensuring the health and wellbeing of individuals requiring acute and post-acute services. Key Responsibilities:- Lead and inspire a multidisciplinary team, promoting a culture of excellence and professional growth.
- Ensure safe and effective patient-centered care for individuals transitioning from hospital to home.
- Collaborate with healthcare teams to ensure smooth transitions and timely access to care across the healthcare journey.
- Champion evidence-based practice, quality improvement, and service development initiatives to enhance patient outcomes.
- Registered Nurse (Division 1) with current AHPRA registration and relevant postgraduate qualifications.
- At least 3 years' clinical experience in acute, post-acute, or Hospital in the Home care.
- Confident in applying advanced nursing skills in complex situations.
