Key Responsibilities:
  • Lead and inspire a multidisciplinary team, promoting a culture of excellence and professional growth.
  • Ensure safe and effective patient-centered care for individuals transitioning from hospital to home.
  • Collaborate with healthcare teams to ensure smooth transitions and timely access to care across the healthcare journey.
  • Champion evidence-based practice, quality improvement, and service development initiatives to enhance patient outcomes.
Requirements:
  • Registered Nurse (Division 1) with current AHPRA registration and relevant postgraduate qualifications.
  • At least 3 years' clinical experience in acute, post-acute, or Hospital in the Home care.
  • Confident in applying advanced nursing skills in complex situations.
