About the Role

We are seeking an experienced Support Worker to join our team at Mission Australia, working with individuals who have experienced severe and recurring mental illness.

The Housing and Accommodation Support Initiative (HASI) aims to provide individualized support to clients, enhancing their housing, community participation, and overall wellbeing.

Key Responsibilities
  • Respond to referrals from internal and external support services, providing outreach services to clients and creating individualized support plans.
  • Develop and maintain relationships with internal and external stakeholders, including service partners and government agencies.
  • Administer internal and external reports, including risk assessments, individualized support plans, and program promotion.
Requirements for Success
  • Relevant experience or qualifications in community services, with a focus on case management and mental health support.
  • Demonstrated experience working with individuals with complex mental health issues, building rapport and advocating for clients.
  • Excellent communication and interpersonal skills, with the ability to liaise with a wide range of stakeholders.
  • Computer literacy, preferably using Windows, and a First Aid certificate.
  • Willingness to undertake relevant training opportunities for professional development.
