Our team leaders play a crucial role in leading and energising teams to deliver exceptional service, building strong relationships with clients that are at the heart of everything we do.
Working closely with our Client Service Managers, our team leaders partner with clients to develop and manage care teams that promote choice, control and confidence under the NDIS framework.
We're passionate about working with people with complex support needs, including 24/7 assistance. Our team leaders are vital in this process, building tailored teams around each client based on their unique needs and preferences.
Our clients may have physical disabilities, acquired brain injuries, neurological conditions, intellectual disabilities or autism. We're privileged to be able to provide essential support needs for many.
The role of a Coordinator is centred around exceptional client relationship management and creating effective support teams. Together with our Client Service Managers, you'll work with clients in their homes or shared accommodation settings to develop and manage teams that support their goals.
Coordinator roles can vary across different settings - you may manage a small caseload of individual clients, a shared or unique supported independent living home, or an apartment complex with up to 10 clients sharing some supports.
All our roles involve built-in support work, maintaining close relationships and connections with our clients' ongoing support needs. The proportion of time spent 'coordinating' and 'on shift' varies depending on the setting and clients.
You'll work as part of a close-knit team in your region in these flexible roles.
Key Responsibilities:- Lead a team of Assistants to provide great service to clients.
- Develop and maintain positive relationships with clients and their loved ones.
- Work collaboratively with clients to achieve positive outcomes.
- Partner with Client Service Managers to maintain client team rosters and recruit team members.
- Provide ongoing support, mentoring and client-specific training for staff.
- Maintain accurate client records and monitor funded support budgets.
- Contribute to policy improvements and participate in our on-call after-hours coordinator roster.
To succeed in this role, you will need:
- Proven experience in disability, health or community services, including direct client support and team coordination.
- A passion for client-focused care and collaboration.
- Excellent communication and interpersonal skills.
- Tech-savviness and comfort with using cloud-based apps.
- Relevant qualifications and/or experience in Community Services/Case Management.
- A full Victorian driver's licence and suitable vehicle.
