Clinical Nurse Specialist

About the Role
We are seeking a highly skilled Registered Nurse to join our dedicated health services team at Integratedliving Australia. As a Registered Nurse, you will play a vital role in promoting the wellbeing of our aged and vulnerable clients, enabling them to live independent lives in their own community.
Key Responsibilities
- Provide direct client care through health, wellbeing and/or clinical services based on an established care plan.
- Work in collaboration with other health professionals to assess and ascertain clients' needs and goals, and to develop service recommendations within defined funding parameters.
- Monitor health outcomes via Telehealth and provide health promotion and health education to clients and carers.
- Deliver our award-winning Wellness for Independence programs in person and virtually.
Requirements
- Understanding of community nursing and the principles of primary health care, including providing services in a consumer-directed care framework.
- Community, acute and aged care experience of 1 or more years.
- Experience working as an independent clinical practitioner or in a team.
- Proficient in the use of electronic technology, including smartphones, tablets, laptops, email, internet and client data management systems.
- Registered with the Australian Health Practitioners' Regulation Authority (AHPRA).
