About the Role
We are seeking a skilled and dedicated Registered Nurse/Endorsed Enrolled Nurse to join our team at Integratedliving Australia. As a community nurse, you will be part of a dedicated health services team focused on promoting the wellbeing of our aged and vulnerable clients, to live independent lives in their own community.
Key Responsibilities
- Provide direct client care through health, wellbeing and/or clinical services based on an established care plan.
- Work in collaboration with other health professionals to assess and ascertain clients' needs and goals and to develop service recommendations within defined funding parameters.
- Provide health promotion and health education to clients and carers.
- Monitor health outcomes via Telehealth.
- Provide direct care services across a range of different funding programs.
Requirements
- Understanding of community nursing and the principles of primary health care including providing services in a consumer directed care framework.
- Community, acute and aged care experience of 1 or more years.
- Experience working as an independent clinical practitioner or in a team.
- Proficient in the use of electronic technology including smart phones, tablets, laptops, email, internet and client data management systems.
- Registered with the Australian Health Practitioners' Regulation Authority (AHPRA).
