We are seeking a compassionate Registered Nurse to join our team in Hobart. As a Community RN, you will deliver in-home clinical care that supports ageing well. You will assess, develop, and evaluate individual nursing care plans, focusing on client goals and independence, with support from a multidisciplinary team.
Your role will involve delivering high-quality care that directly improves the well-being of older Australians in your community. You will work independently, providing health education and promotion, wellness for independence, telehealth services, direct care services, and person-centered care.
You will be required to have a Division One RN qualification, current Police Check, CPR, Blue Card, and Driver's Licence. A minimum of 1 year experience in community, acute, or aged care is also required.

Advanced skills in areas like chronic disease, wound care, palliative care, and diabetes management
Strong understanding of primary health care and consumer-directed care frameworks

Excellent organisation, attention to detail, and ability to work both independently and collaboratively
