Job Summary:

The Professional Care Pharmacist will play a key role in providing exceptional patient care and health services to our community. As a member of our team, you will be responsible for delivering high-quality healthcare solutions to patients, using your expertise in medication management and patient counseling.

Key Responsibilities:

  • Provide comprehensive patient counseling and education on medication use and management.
  • Develop and implement effective medication therapy plans to improve patient outcomes.
  • Collaborate with healthcare professionals to ensure seamless patient care and coordination.
  • Conduct health assessments and provide recommendations for disease prevention and management.
  • Stay up-to-date with the latest medication information and guidelines to provide evidence-based care.
  • Participate in quality improvement initiatives to enhance patient care and services.

Requirements:

  • Bachelor of Pharmacy (or equivalent) degree.
  • Current General Registration as a pharmacist (without conditions or undertakings) with AHPRA.
  • Pharmacist Scope of Practice training.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
