Case Manager

The Case Manager – AOD Outpost is a critical role within Launch Housing, responsible for providing support to clients experiencing addiction and/or dependence. This position contributes to a world-class harm reduction program, working closely with clients to identify and achieve their goals.
Key Responsibilities- Support a self-determined caseload of voluntary clients to identify and achieve AOD related goals.
- Complete AOD screening/assessment tools, and support clients in accessing residential and community-based support services.
- Develop Individual Treatment Plans (ITP's) collaboratively with clients.
- Support complex clients through the life of their treatment plan.
- Provide harm-reduction based information sessions to at risk clients.
- Provide education and training sessions to staff of Launch Housing.
- Provide secondary consults to Launch Housing staff and external agencies.
- Contribute to the ongoing operation of a (secondary) Needle and Syringe Program (NSP), and a world-class harm-reduction program.
- A relevant tertiary qualification which meets the minimum standard qualifications to work in the AOD Sector (e.g. Certificate IV in AOD or AOD competencies) or a willingness to obtain these qualifications.
- Demonstrated experience in client facing human services, social work or other relevant discipline, with a preference for an understanding of housing and homelessness policy, context and systems.
- An understanding of AOD treatments and interventions, including knowledge of the different types of public AOD services available.
- An understanding of AOD theories, including harm minimisation and problematic substance combinations.
- An understanding of trauma informed practice and the individual and structural relationships between trauma, substance dependence, and homelessness.
- High degree of initiative, communication, and analytical skills, including well-developed organisational and time-management skills.
- Experience or training in facilitating group work (desirable).
- Full Victorian Drivers Licence (mandatory).
