Job Description

We are seeking an experienced Senior Aged Care Coordinator to join our team at Myhomecare in Townsville. This is a leadership role that requires strong communication and organizational skills.

Key Responsibilities:

  • Maintain high standards of care provided by Myhomecare staff and providers.
  • Lead from the front as a Senior Aged Care Coordinator, utilizing customer service and aged care management skills to drive quality services and community engagement.
  • Develop, monitor, and reassess care plans in consultation with clients, their families, and healthcare professionals.
  • Collaborate with Nurses, Regional Managers, and Portfolio Managers to develop and shape service offerings, ensuring the highest standard of care delivery.
  • Manage and develop a team including Assistant Care Managers and support staff.
  • Monitor care package budgets and ensure clients understand service fees.
  • Ensure compliance with relevant legislative and industry standards.
  • Proactively engage in continuous improvement across practice and service delivery.

Requirements:

  • Recent experience in case management in Aged Care or Community Services sector.
  • Clinical experience preferred.
  • Knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards desirable.
  • Frontline management skills.
  • Current First Aid Certificate and current CPR certificate.
  • The right to work in Australia.
  • A current driver's license, registration, and access to a reliable vehicle with comprehensive insurance.
  • National Police Clearance.

Salary Information: The estimated annual salary for this position is AU$85,000 - AU$110,000, depending on experience.
