Care Coordinator for Maternal and Child Health

Our organization's Australian Family Partnership Program is a groundbreaking initiative aimed at enhancing maternal and child health. This voluntary, evidence-based home visitation program seeks to make a positive impact on the lives of eligible women and their families.
The position offers an exceptional opportunity for Registered Nurses/Midwives and Aboriginal Family Partnership Workers to work closely with clients, developing long-term relationships that empower them to recognize their strengths, build skills and confidence, and access essential services.
About the Role:
- Build strong connections with clients through regular home visits
- Support clients in identifying and leveraging their strengths
- Facilitate access to vital services and resources
Requirements:
- Registered Nurse or Midwife qualification
- Experience working with diverse communities, particularly Aboriginal populations
- Excellent communication and interpersonal skills
