About the RoleThe Australian Family Partnership Program is a voluntary home visitation program that aims to improve maternal and child health. As a Nurse Home Visitor, you will work closely with eligible women from early pregnancy until their child turns two, providing education and support on maternal and child health and referring them to relevant services when needed.
Key Responsibilities- Establish strong relationships with clients, identifying their needs and developing tailored support plans.
