Aged Or Disabled Care Provider

We are seeking a compassionate and skilled Aged Or Disabled Care Provider to deliver high-quality care and support services to our clients.

Key Responsibilities:

  • Assist clients with daily living activities such as personal care, meal preparation, medication management, manual handling, attending appointments, and household tasks
  • Support clients in community participation and social activities
  • Encourage and promote independence and self-determination
  • Assess clients' needs and develop tailored plans and programs
  • Interview clients and assess their difficulties
  • Monitor and report on client progress
  • Build strong relationships with clients, families, and relevant service providers
  • Collaborate with colleagues and demonstrate person-centered care principles

Requirements:

  • Certificate III or IV in Aged Care, Disability Services, or individual support
  • At least 2-3 years of relevant experience
  • Australian citizenship or permanent residence is highly desirable
