Job Role

The Family Wellbeing Service Practitioner will partner with families to empower them and build their capacity to make effective change and overcome identified challenges.

This role involves working closely with families in their homes, translating initial engagement into a strong ongoing partnership.

The practitioner will facilitate interventions that increase family awareness and insight into their history and current challenges, enabling families to self-identify key issues and lead their own change.

Key Responsibilities
  • Meet performance targets through regular review and analysis.
  • Provide high-quality practice that aligns with our organisational values and vision.
  • Maintain accurate and up-to-date records that adhere to operational requirements.
  • Engage clients in a positive and assertive manner, building trust and rapport through outreach efforts.
  • Conduct client-led assessments and develop Care Plans that address individual goals, including those of the entire family where relevant.
  • Deliver targeted interventions and facilitate group meetings to achieve client objectives.
  • Prioritise quality management through the use of LogiQC systems and MobNet tasks.
  • Foster productive working relationships at all levels, promoting cooperation, trust, and respect while valuing diversity and differences.
